Student Self-Certification

Each SD129 student will be required to have a self-certification on file for the 2021-22 school year, completed by their parent or guardian. You will need to submit one form for each child in your household on or before August 16th.
This self-certification confirms (among other things) that students will not attend school while experiencing COVID-like symptoms. A link to the student self-certification form can be found below:
WHAT DOES SELF-CERTIFICATION MEAN?
Staff and students will certify at the beginning of the year that they will not come to work or school while experiencing COVID-like symptoms, which include:
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea
